About Our Plans

The Dental Clinique Wellness Plan is a membership-based dental wellness plan in Arlington Heights, IL that provides the quality care you deserve at a price you can afford. Members pay an annual fee to receive regular exams, cleanings and X-rays along with access to significantly reduced rates on all other restorative and cosmetic dental procedures performed in our dental office in Arlington Heights, IL. Plus, the plan offers many benefits including no annual caps, no limits, and no waiting periods. This provides for quick access to the care you need!

Program exclusions and limitations: This is a wellness plan, not dental insurance. It cannot be combined with any other insurance. It is only valid at our Arlington Heights, IL location; care from other providers and specialists is not included. Plan fees are subject to change. For complete details, see Plan Agreement or Plan Terms and Conditions. Children 13 or younger. If periodontal infection is present, a periodontal plan may be required at an additional cost.

*This basic Adult Plan is designed for patients who do not have infection present in the mouth. If periodontal infection is present, the alternative periodontal plan will be required.

• Purchase the Dental Clinique Wellness Plan for each of your qualified team members on an annual basis. As new employees come on board you may add additional plans. See Plan Terms and Conditions for more information on adding and removing employees from the plan.
• If your company is unable to cover the cost of Dental Clinique Wellness Plan for your employees, consider offering the plan as a recommendation or supplement to any other benefit you may be offering. We will be happy to work directly with your employees to set up the plan to meet their dental care needs. Many of our employers will even cover a percentage of the total cost through an employee reimbursement program. We would be happy to discuss options with you on a one-on-one basis.
